Home For Me offers a “hospital to home” service that allows clients being discharged from the hospital a comforting advantage in a somewhat scary process.
The Home For Me “hospital to home” service allows clients, as they are being discharged from the hospital to have professional support, comfort and care. The Home For Me caregiver picks up the client from the hospital, goes over the discharge instructions, picks up prescriptions and assures that the client is comfortably situated back at home.
A flat fee of $120 for three hours of service, covers a complete review of the discharge paperwork, prescription pickup and care coordination. If the client continues with Home For Me service, they will receive 10% off their first week of service.
